FAQs

1. Register & pre-qualify. Register and take our quick Eligibility Survey. You will immediately find out if your condition may qualify you to receive your Florida Medical Marijuana Recommendation.

2. Schedule your appointment. Contact us and one of our friendly team members will schedule the next available appointment for your exam with one of our certified physicians. A credit card is required to book your exam with the physician.

3. Receive your registry id#. Once your exam is completed and you have met the Florida requirements, your ChoiceMedGroup physician will enter the order with the Office of Medical Marijuana Use. You will then receive your Registry ID# and with that you can receive your Medical Marijuana Card from the State of Florida.
Yes, after your in-person examination, your ChoiceMedGroup physician will determine if you qualify for medical marijuana. The attending physician will make the recommendation and input your order information into the Medical Marijuana Use Registry.
Please bring your Florida Driver’s license or ID card showing that you are a resident of the of Florida. Your medical records are required for the visit. We prefer you fill out our online Eligibility Survey. Once you have completed and submitted the survey, one of our Patient Care team members will review the information. If you have a qualifying condition, you will then receive an email with the secure forms necessary for your visit. These secure forms can be easily completed online enabling ChoiceMedGroup to electronically request your medical records prior to your visit. Or if you prefer, you may fax your records directly to ChoiceMedGroup from your provider’s office.
We need supporting medical records of your diagnosis of a qualifying condition under Florida law. Usually the most recent office visit from your primary or specialist treating physician, will have a summary of your diagnosis and treatments up to date for your indicated condition.
Unfortunately no. There are no insurance carriers that pay for Medical Marijuana related services or medicines.
Though referrals can help with the process, we do not require referrals from another physician. However, you will need to have a previous diagnosis as listed above, and been treated by a physician or specialist for a qualifying condition. ChoiceMedGroup requires patient provide copies of their medical record to support their diagnosis.
Pre-qualification means a ChoiceMedGroup Patient Care team member will review your Eligibility Survey or talk to you on the phone and let you know if your condition would likely qualify you to be certified to receive medical cannabis. There is no charge for this service.

Qualification is the process through which a patient becomes certified to receive the Florida Medical Marijuana Card. You must be seen in-person by a physician in our doctor’s office. There is a charge for this physician consultation and office visit. To see a list of our fees, click here.

Certified — Patients with a qualifying condition who have seen our physician are certified and added to the Florida Medical Marijuana Use Registry in accordance with Florida law.

Florida law has several requirements for patients to be eligible to receive medical marijuana. 
A patient must be a Florida resident.
A patient must have been diagnosed with a qualifying condition.
If patient is under the age of 18, a patient must have a second physician agree to the use of low-THC cannabis or medical cannabis in order to obtain an order from a qualified physician.
A patient must have tried other treatments without success.
An ordering physician must determine the risks of using medical marijuana are reasonable in light of the benefit(s) to the patient.
A patient must be registered with the "Florida Office of Medical Marijuana Use Registry by a qualified ordering physician.
Yes. If patient is under the age of 18, a patient must have a second physician agree to the use of low-THC cannabis or medical cannabis in order to obtain an order from a qualified physician.
After your examination, your ChoiceMedGroup physician inputs your order information into the Florida Medical Marijuana Use Registry, you or your legal representative will then receive an email from the Office of Medical Marijuana Use with your user name and instructions of how to complete the process. After completion of the application process, you will receive an email with your Compassionate Use Registry Identification Card.
Once you have received the email with your Compassionate Use Registry Identification Card, you or your legal representative will then be able to contact one of the licensed dispensing organizations and fill the order.
Florida rule 64-4.011, F.A.C. requires all patients and legal representatives to have a Medical Marijuana Use Registry identification card to obtain medical marijuana, or a marijuana delivery device from a licensed dispensing organization.
An object used, intended for use, or designed for use in preparing, storing, ingesting, inhaling, or otherwise introducing marijuana into the human body, and which is dispensed from a medical marijuana treatment center for medical use by a qualified patient.
"Low-THC cannabis" means a plant of the genus Cannabis, the dried flowers of which contain 0.8 percent or less of tetrahydrocannabinol and more than 10 percent of cannabidiol weight for weight; the seeds thereof; the resin extracted from any part of such plant; or any compound, manufacture, salt, derivative, mixture, or preparation of such plant or its seeds or resin that is dispensed from a medical marijuana treatment center.

"Marijuana" means all parts of any plant of the genus Cannabis, whether growing or not; the seeds thereof; the resin extracted from any part of the plant; and every compound, manufacture, salt, derivative, mixture, or preparation of the plant or its seeds or resin, including low-THC cannabis, which are dispensed from a medical marijuana treatment center for medical use by a qualified patient.

For your initial appointment, you should arrive at least 10-15 minutes early. We advise you to complete (read, initial and sign) the online intake forms you received via email. If you haven’t completed your Online Intake forms, you’ll need to arrive at least 40-50 minutes early to complete the necessary forms.

When you arrive, we’ll verify your Intake forms are completed. We will then collect payment for your visit (prior to services being rendered).

You’ll then be taken back and one of our Patient Care team members will take your vitals. The doctor will then perform an in person examination, discuss your condition, previous treatments, any concerns, answer questions and develop a treatment plan. Once your exam is complete, we’ll schedule your follow up appointment before you leave.
The first visit requires the doctor to review your medical history and discuss your options, the benefits and expectations of a medical cannabis treatment plan. In order to allow adequate time for you and the doctor, please allow for 45 minutes to an hour. We can provide a written excuse if needed for work or school.
By law patients must be seen in person a minimum of once every 210 days (30 weeks).

For patients who are inexperienced in cannabis use or are extremely debilitated, the physician has the right and responsibility to see the patient on a more regular basis.

Because each patient has their own unique needs and concerns, during your initial visit the attending physician will create a follow up visit schedule specific to your needs.

For example, patients with a simplified care plan, might be placed on a schedule that requires an online follow up Televisit in 70 days. Or for patients with a more complex care plan, an online follow up Televisit might be scheduled more frequently.
Medical marijuana is dispensed by a state approved Medical Marijuana Treatment Center (MMTC). Currently Florida has seven authorized dispensing organizations: CHT Medical, The Green Solution, Trulieve, Surterra Therapeutics, Modern Health Concepts, Knox Medical, and GrowHealthy.
